This funding Funding provides community outreach, increased testing and wraparound supports and work work in partnership with Ontario Health, public health units, municipalities, and other community partners to deliver key interventions for the province’s hardest-hit neighbourhoods.

  • Community Ambassadors to provide door-to-door outreach, promoting access to available services and supports; and
  • Developing targeted and culturally appropriate communications to ensure community members and leaders have the information needed to combat misinformation and myths.
  • Health promotion and prevention with a focus on chronic disease prevention, improved healthcare access, mental health and healthy living support.

Increased access to testing, including those tailored to the unique needs our communities and expanded and flexible hours of operation.

  • In addition, we continue to develop community partnerships to understand and support addressing barriers.

Wraparound supports using a case management approach to connect individuals and families with available services, such as where they can get prevention testing, counselling, find health services and programs to promote education, activity, positive change, reduce isolation and improve health outcomes.

  • Increases access to care; and
  • Reduction in barriers and gaps across health services.
  • Improves chronic disease behaviours and health outcomes.

We serve community members up to 40 years old.
