Roots Community Services, LAMP Community Health Centre and the Canadian Mental Health Association (CMHA) Peel Dufferin are partnering to offer this initiative which was being designed with the holistic health of the individual as the primary focus. This ensures that services are easily accessible and closer to home for Black, African and Caribbean (BAC) communities in Peel.

The Hub was co-designed by Peel residents using an Afrocentric lens, and is informed by Ontario’s Black Health Plan, which has on overarching focus on equity and social justice.

While our focus is on the Black, African and Caribbean communities, we welcome everyone who can benefit from the culturally sensitive and responsive services we offer.

We prioritize individuals and families who do not have another family doctor.
– You can access healthcare services under the HUB even if you do not have healthcare coverage (such as OHIP or Interim Federal Health Program -IFHP).

We offer translation and interpretation services as needed. Please feel free to reach out to us directly for more information or assistance.
