About Primary Health Care
We strive to provide comprehensive primary health care services through our team that includes family physicians, a nurse practitioner, a registered practical nurse and administrative/reception staff.
Through our interdisciplinary approach, our goal is to provide holistic care to those living within our community with a focus on those who may face barriers to accessing care. Our team strives to be inclusive and works within the framework of the social determinants of health with a focus on prevention and health education.

The Black Health and Social Services (BHSS) Hub
Introducing the Black Health and Social Services (BHSS) Hub, bolstering support for Black communities in the region.
The BHSS Hub is a joint initiative between Roots Community Services, Canadian Mental Health Association Peel Dufferin, and LAMP Community Health Centre.

- Appointments for forms, injections, pregnancy care, physical exams, PAPS, well-baby/well-child care, or any other specialized visit must still be booked by calling the appropriate reception desk, as follows. Please do not use the online booking feature if you require any of the above-mentioned services.

Our clients range from newborn to elderly and live in the LAMP CHC catchment area.
For individuals who are uninsured without status (current immigration documents) and for gender affirming care there is an expanded catchment area. We unfortunately are unable to accommodate students or those on work visas.
We offer translation and interpretation services as needed. Please feel free to reach out to us directly for more information or assistance.
