High Priority Communities Strategy
Unattached High Priority Community Care is a project funded by the Ontario government which aims to address health disparities in marginalized communities.
This funding Funding provides community outreach, increased testing and wraparound supports and work work in partnership with Ontario Health, public health units, municipalities, and other community partners to deliver key interventions for the province’s hardest-hit neighbourhoods.

About The Unattached High Priority Community Care Project
This project focuses on:
Tailored community outreach and engagement to enhance awareness of the services and supports available, including:
- Community Ambassadors to provide door-to-door outreach, promoting access to available services and supports; and
- Developing targeted and culturally appropriate communications to ensure community members and leaders have the information needed to combat misinformation and myths.
- Health promotion and prevention with a focus on chronic disease prevention, improved healthcare access, mental health and healthy living support.
Increased access to testing, including those tailored to the unique needs our communities and expanded and flexible hours of operation.
- In addition, we continue to develop community partnerships to understand and support addressing barriers.
Wraparound supports using a case management approach to connect individuals and families with available services, such as where they can get prevention testing, counselling, find health services and programs to promote education, activity, positive change, reduce isolation and improve health outcomes.
