Please read the information below – there are a few rules
If appointments are booked online but do not meet criteria, the appointment will be cancelled. We will make our best efforts to contact you but in the event that we cannot, we will re-direct on arrival as appropriate.
To Book Online:
- You must already be an active client of the service or Provider you are booking with.
- Appointments for forms, injections, pregnancy care, well-baby/well-child care, or any other specialized visit must still be booked by calling the appropriate reception desk, as follows:
LAMP General Reception: 416-252-6471
LAMP Primary Care: 416-252-6475
West Toronto Diabetes Education Program: 416-252-1928
- There may be additional restrictions when using online booking not listed which can result in cancellation – your Provider or a staff may speak to you directly (including frequency or # of bookings)
- New patients cannot book appointments online at this time
